During your hospital stay and certainly prior to your discharge, you will be provided with a questionnaire to complete. If you do not receive such a questionnaire form during your stay, it may also be provided from the Admissions Department during your discharge. There are similar questionnaires that pertain to services offered at OCSC Outpatient and Emergency Departments. Your privacy will be protected and you will remain anonymous while filling this questionnaire and your evaluation will have no bearing on your medical treatment.
